Adult Foster Care/ Adult Family Care (AFC) is a family and residential care program ideal for individuals deemed unsafe to live alone but do not need the support level provided in a group residential setting. Individuals approved for AFC benefit from a care program involving 24-hour supervision to assist with daily living activities.
With AFC, approved members will live with committed, responsible, skilled, and trained caregivers who can provide personal care, meal preparation, and companionship, go along with them to doctor appointments, transportation, health monitoring, and help with medication reminders. AFC members have the option to reside with a host family or remain at home with their family members, who will provide daily care and support.
